Do you consent to your child taking the lateral flow Covid-19 tests in school?
As part of the return to school this September, we are required to offer all students two lateral flow tests. The first was the day before they returned. The second will take place during school time. We would like to offer the second test to all consenting students, whether or not they came for the drop in test before returning to school. For those who did not attend the first test, we need to ask for consent again.
This is a voluntary programme but we encourage you to take part if you can, as it will help get the year off to a safe start. The test (ie. nose swab for approximately 10 seconds per nostril) will be undertaken by the student themselves, under the supervision of a trained adult.

Who should complete the form?
If your child did not attend the first test at the drop in sessions.
Who does not need to complete the form
If your child attended the drop in tests, we already have your consent recorded.
